Probable Solution:

The database administrator needs to run the SQL script that creates the RDBMS tables in the datastore which are used by the RDBMS security store. A set of SQL scripts for creating and removing these tables for each supported RDBMS system is available in WL_HOME/server/lib
During the process of configuring the RDBMS security store via the Configuration Wizard, you are presented with the option of testing the database connection. Oracle strongly recommends using this option because it can verify that the connection is set up properly. If there were a problem with the database connection, you might not be able subsequently to boot the domain if the security providers that control access to that domain are unable to obtain the necessary security policies.

Create RDBMS Tables in the Security Datastore

Prior to booting the domain, the database administrator needs to run the SQL script that creates the RDBMS tables in the datastore used by the RDBMS security store. A set of SQL scripts for creating these tables for, and also removing them from, each supported RDBMS system is available in the following WebLogic Server installation directory:

When running the appropriate SQL script for the database serving as the RDBMS security store, be sure to specify the same connection properties, including the credentials of the user who has access, the database URL, etc., as specified for that RDBMS during domain creation.

after that i checked environment variables:

it’s correct, then checked /etc/host file

bingo! someone maybe system administrator or other dba commented this machine hostname and ip, hence we can’t start database, uncomment it and everything goes correct.

On running adcfgclone.pl on 11.2.0.3 RDBMS ORACLE_HOME, it got errored out with the message below.

Reason:
After upgrading you database from 10g to 11g , perl utility has been upgraded from 5.8.3 to 5.10.0.


Solution:
This is a known issue and now documented in My Oracle Support Article ADPreclone Fails When Can't Find Strict.PM Following Database Upgrade to 11gR2 [ID 1139403.1].

11.2.0.3 ships with perl 5.10.0. So you need to change the values of context variables pointing to PERL5LIB and ADPERLPRG inside the context file.

vi $CONTEXT_FILE
Global replace 5.8.3 with 5.10.0
Save
cd $ORACLE_HOME/appsutil/template
vi adxdbctx.tmp
Global replace 5.8.3 with 5.10.0

Run adcfgclone.pl, or adconfig or adpreclone.pl and it will work.

Issue:
On Oracle Applications R12 when checking the top processes on the OS level for the middle tier, you find that forms process (frmweb) almost consumes 100% of the CPU.

Cause:
The root cause of the issue is that returning rows from LOVs in core forms causes the forms process to grow up into memory depending on the number of rows returned.
When an end user login to forms and start working with LOV within core forms sometimes and according to the search criteria that the user will provide to filter the results in LOV, it may fetch huge numbers of records in which causes the frmweb process to grow very large, and in extreme cases this can even lock up the current process or even the whole machine.

So when executing a LOV query, every row is fetched into memory on the middle tier, the frmweb process can get extremely large, and the larger it gets the more likely it is to start paging.
Eventually it starts consuming excessive CPU just paging the process in and out of memory, which is probably what you can see here in this case as the amount of memory consumed when the LOV records are fetched into memory obviously depends on the amount of data in each record.

Solution:
FORMS_RECORD_GROUP_MAX to 10000 or 20000 or 30000.
FORMS_CATCHTERM=0

Apache / iAS 
For 11i and earlier versions:
- shutdown iAS server
- go to $OA_HTML (for 11.5.9) or $COMMON_TOP (for 11.5.10.x) directory
- backup the directory _pages and delete its contents by running for instance:

  rm -rf $COMMON_TOP/_pages/*

- for modplsql caches remove contents of  $IAS_ORACLE_HOME/Apache/modplsql/cache directory
- restart iAS server


To clear middle tier cache in release 12:

- go to "Functional Administrator" responsibility
- select Core Services => Caching Framework => Global Configuration => Clear cache
(please review Note 759038.1 for details).
In case you have login issue after accidentally cleared the _pages instead of using the method above for r12
please review Note 433386.1 to recompile jsp files.

Cabo
Images and style sheets can be corrupted or out of sync in the cabo caches,
you may need to clear the related directories after backup:

$OA_HTML/cabo/images/cache
$OA_HTML/cabo/styles/cache


Web Browser

for Internet Explorer:
- go to menu Tools => Internet Options,
- select 'General' tab,
- click on button 'Delete Files' in 'Temporary Internet files' area
  or
- click on button 'Delete...' then in 'Delete Browsing History' pop-up window click on 'Delete Files...'
- close all IE windows and restart new browser session.

for Mozilla Firefox:
- go to menu Tools
- select 'Clear Private Data...' or 'Clear Recent History...' then check 'Cache'
  or
- go to menu Edit or Tools
- select Preferences or Options.
- expand the 'Advance' options and choose 'Cache' or Privacy
- click the button called 'Clear Cache'.

for Safari:
- go to Safari menu
- select Empty Cache => Click 'Empty' in the dialogue box
for Netscape:
- go to menu Edit => Preferences,
- choose 'Cache' in 'Advanced' category,
- click on buttons 'Clear Memory Cache' and 'Clear Disk Cache'.
- close all Netscape windows and restart new browser session.

Jinitiator

Two possibilities depending of the Jinitiator version:

for 1.1.8.x versions:- delete all files in directory:

C:\Program Files\Oracle\Jinitiator <version>\jcache\

for 1.3.1.x versions:
- go to Start => Parameters => Control Panel
- double-click on "Jinitiator <version>" icon
- in the new pop-up window, click on "Cache" tab
- click on "Clear Jar Cache " button. On prompt, click Yes.

(you can also delete directly all files under directory:
C:\Documents and Settings\<your user name>\Oracle Jar Cache)


Java
Java/JRE plug-in (Windows)- go to Start => Parameters => Control Panel
- double-click on 'Java' icon
- in the new pop-up window, click on 'General' tab
- click on 'Setting...' button in 'Temporary Internet files' area then click on 'Delete Files...' button
  (you can select Applets, Applications or other files)

JVM

- go to responsibility "Functional Administrator"
- click on "Core Services" tab then the "Caching Framework" sub-tab
- click on "Global Configuration" link then click on "Clear all cache" button to clear all of the Java Cache's


Portal
- go to the url: http://<host.domain>:<port>/pls/admin_/gateway.htm or cache.htm
- click on "Cache Settings" option and note the "Cache Directory"
- go to this directory and delete all the cache files in the directory and sub directories

WebADI / BNE cache
For instance when enabling BNE log
- go to url: http://<host.domain>:<port>/oa_servlets/oracle.apps.bne.framework.BneAdminServlet
- click on the "clear-cache" link
- at the bottom of the page you should see 'Cache Cleared'

Issue Reported.
Got with error below when try to access forms soon after logging into Oracle Applications.


Please implement below steps, to overcome the issue.

1.    Go to Control Panel.    Start --> Control Panel

2.    Select Java (Java Control Panel) from the list. 

3.    Once you have opened the “Java Control Panel”, click on the “Security” tab. Change the Java security from “High” to “Medium” by moving the pointer to “Medium". Then hit Apply and OK.

4.    Close all the browser sessions, launch browser and try to access Oracle Applications again.

Issue Reported.
Got with error below soon after logging into the Oracle Applications.
 

Solution:
The users where experiencing this change soon after the Java Upgrade on the Application Tier.

Go to the Java Control Panel (On Windows Click Start and then Configure Java)
Click on the Security tab
Click on the Edit Site List button
Click Add in the Exception Site List window
Click OK
